The present letter is to notify to the concerned parties and corresponding authorities that Julio
Torres, as our Field Inspector has performed an inspection of the foundations for this project.
The inspection included a visual review of the foundations based on the compaction performed
by Dynatech Engineering Corp., dated March 8, 2022 and the Foundations Recommendations
specified by B3 Material Testing Engineering on May 26, 2021 under Project No. 21-0259.
The topsoil around the proposed foundations was removed to the necessary depth, backfilled
with a mixture of sand, limerock and limesand fill in compacted layers of approximately six (6)
inches and compacted per AASHTO T-180 to a minimum compaction of 98% of the optimum
dry density. We can attest that the Foundation Recommendations set forth on the Geotechnical
Report have been followed accordingly and recommend your approval of the foundations and to
allow the casting of the Foundations per approved Structural Plans.

FOUNDATION RECOMMENDATIONS:
Our recommendations are based on the information provided from the client as to the type of structure
planned and on our subsurface investigation performed on the proposed site. Our recommerdations
are as follows:
1. Clear entire building area plus 6-0" outside the perimeter of construction am remove all topsoil,
and unsuitable subsurface material to the necessary depth. We anticipate an average clearanct-
depth of approximately six (6) inches.
2. Compact cleared area to a minimum compaction of 98% of the ❑ptimu:n dry denctty as per
AASHTO T-180. Verify densification procedures by taking an adequate of field density
compaction tests. The cleared area should be inspected prior to the commencement of the
backfilling operation to ensure that all the unsuitable material has been removad.
3. Backfill building area, plus 6-0" outside the perimeter of the structure to the required elevation with
a clean mixture of sand, lime rock and lime sand fill (or approved fill material) in compacted layers
not to exceed 10 inches of thickness. Compact each layer to a minimum of 98% of the optimum
dry density as per AASHTO T180.
4. Excavate footing trenches to the required depth from the ground elevation. Compact the bottom of
the footing trench to a minimum compaction of 98% of the optimum dry density as per AASHTO
T-180. Verify densification procedures by taking an adequate number of field density compaction
tests.
Special Considerations:
a) The subgrade at the design elevation should be observed by a geotechnical engineer and any
topsoil, organic, unsuitable, or deleterious material removed. Proof rolling of the resultant
subgrade should be performed to locate objectionable soils that should be removed. During
the proof rolling procedure, the stripped soil surface is rolled with the heaviest piece of
construction equipment available at the site, such as a heavily loaded tandem axle dump truck
having a gross weight of not less than 25 tons. Areas exhibiting deflection or rutting over 1.25
in (32 mm) should be removed and the proof rolling continued until all unsuitable soils have
been located and removed or improved in -place. However, the on -site inspector should ensure
that the finished subgrade does not exhibit more than 0.5 inch (12.5 mm) of rutting.
b) Floor Slab: After the existing building is demolished and the debris carter off -site, all buried
structures and utilities should be removed from the planned building area plus 5-foot-wide
perimeter. Obtain demolition permit after conducting asbestos surveys before commencement
of demolition activities. The cleared site should be graded and filled to required elevation. Fill
used to raise the building pad should be granular in texture, have not particle size larger than
three inches and have a fines content of no more than 12%.

i. A vapor barrier should be placed over the building pad fill, prior to placement of
concrete for the floor slab. This will tend to minimize the entrance of subgrade moisture
into the slab. Normally a 10-mil film of polyethylene is used for this purpose.
DESIGN RECOMMENDATIONS:
The above foundation recommendation having been achieved and verified, we anticipate that the
foundation and footings may be appropriately proportioned for a safe soil bearing capacity not to exceed
2,500 psf (pounds per square foot). The use of spread footings and single column oads is suggested.
A monolithic slab foundation may also be adopted.
LIMITATIONS OF STUDY
The exploration and evaluation of the foundation conditions reported herein are considered sufficient
in detail and scope to form a reasonable basis for the foundation design. The recommendations
submitted for the proposed construction are based on the available subsurface information and the
preliminary design details furnished by the client.
Any revisions to the plans for the proposed structure from those enumerated in this report should be
brought to the attention of the soil engineer, so that he might determine if changes in the foundation
recommendations are required. If deviations from the noted subsurface conditions are encountered
during construction, they should also be brought to the attention of the soil engineer, so that he can
review the changed conditions ad revise these recommendations, if warranted.
GENERAL QUALIRCA T1ONS
The analysis and recommendations presented in this report are based upon the data obtained from the
soil borings performed at the indicated locations and from any other information discussed in this report.
This report does not reflect any variations that may occur between borings or across the site. Any
statements which appear in this report or on the boring logs regarding odors, color, unusual or
suspicious items or conditions are strictly for the information of the client. In addition, the soils samples
cannot be relied on to accurately reflect the strata variations that usually exist between sampling
locations. The nature and extent of such variations may not become evident until construction. If
variations appear evident, it will be necessary to reevaluate the recommendations of the report. In
addition, it is recommended that B3 Material Testing Engineering, LLC. be retained to perform
construction observation and thereby provide a complete professional geotechnical engineering service
through the observational method. For environmental due to diligence, a phase I and/or phase II
Environmental Site Assessments is recommended.

Pursuant to your request, Dynatech Engineering Corp. (DEC) performed a Compaction Test on
May 3, 2022 at the above referenced project. DEC was authorized to perform a compaction test
only. The purpose of our test was to determine the degree of compaction of the tested layer of
material only. In no way shall a compaction test replace a demucking inspection, soil bearing
capacity determination, or a pad certification. A soil boring test must be performed by client if
not yet done, prior to construction to verify subsoil conditions. Underlying soils below test levels
must be verified to prevent future settlements. (DEC) was not authorized to perform supervision
and certification of the building pad preparation. This is not a pad certification. Our reports must
be provided to all involved parties for their approval.
Our findings are relative to the date and areas of our site work and should not be relied upon to
represent conditions on other areas or dates. Any subsequent site disturbances due to water
erosion, rain, storm gutters discharging at footings, construction activities, excavations,
overgrown, vegetation, traffic and other disturbances will void this test and the site must be re -
compacted and re -tested prior to construction.
Environmental analysis of the soil materials is not part of the scope of services. If environmental
analysis of the soils is required, we can provide a proposal for performing an environmental
analysis of the soil materials. No other analysis is implied or warranted.

GENERAL TERMS AND CONDITIONS
By specific, reference hereto. the Purchaser understands that General Terms and Conditions are hereby made a part of the Service Order and apply without exception. Purchaser
further understands that their signature on this Agreement accepts the terms offered by PESTDEPOT.COM, hereby referred to as "The Company," for good and valuable
consideration receipt of which is hereby acknowledged.
1. If moisture and/or structural conditions which are conducive (o Termites are subsequently found to exist without ground contact, then The Company shall be relieved of any
and all liability.
2. Structural or mechanical defects which result in water leakage in interior areas or through the roof or exterior walls of the premises may destroy the effectiveness of The
Company's treatment, thereby permitting infestation to continue after the dale of Initial treatment. If such a condition is discovered, it Is agreed that the customer will be
responsible for making such repairs as are necessary to correct the structural or mechanical defect(s) immediately upon discovery of such defect. Should a re -treatment of
any affected area(s) be needed, the expense of such will be charged to the customer as a condition of a continuation of the warranty, or warranty may be terminated unless
full re -treatment is purchased by purchaser.
3, The Company's liability shall be terminated should 'fhe Company be prevented from fulfilling its responsibilities under the terms of this Service Order of the Guaranty by
reason of Acts of War, whether declared or undeclared, Acts of any duly constituted Government Authority, strikes, or Acts of God.
4, This Agreement covers the premises diagramed on (he Graph and Specifications Sheet as of the dale of actual treatment, and in the event the premises are structurally
modified, altered, or otherwise changed after the date of initial treatment, this Agreemen( shall terminate unless a prior written agreement shall have been entered Into by the
Purchase for The Company unless alteration or changes in writing and executed by a corporate officer of The Company under the corporate seal.
5. It is understood and agreed between the parties that this Contract, the Graph and Specifications Sheets which bear this Contract Number, and, upon issuance, the Guaranty
constitute the complete agreement between parties and that said agreement may not be changed or altered in any manner, oral or otherwise, by any representative of The
Company unless alteration of change be in writing and executed by a corporate officer of The Company under the corporate seal.
6. This Agreement is a limited control guarantee, which is limited to local lrealment only and does nol cover re-lurnigation after the initial one yearguarentee. The Company shall not be liable for any

A pest control company must give you a written contract prior to any treatment of each wood -destroying organism. It is very
important that you read and understand the contract you are signing. The pest control company is only obligated to follow the
terms of the contract you have signed, regardless of other statements by the company or salesperson. (Note: Contracts for
treatment for new construction can be issued to the builder and provided to you at closing).
BASIC REQUIREMENTS F,QR CQNTRACTS
• The contract must state the common name of the wood -destroying organism to be controlled by the company (e.g. subterranean
termite, powder post beetle). If the contract is for termite control, the contract must clearly state whether Formosan termites are
covered or not.
• Some contracts do not include a treatment at the time the contract is issued, and that should be clearly stated. If a treatment is performed
as part of the contract, the cost for the treatment must be stated. If the treatment is only for certain areas. the contract should clearly
state that it is for "spot treatment" only.
• The contract must state if it is a retreatment only or a retreatment and repair contract. If it is a retreatment and repair contract, carefully
read the sections of the contract that state when repairs will or will not be covered by the contract.
REQUIREMENTS FOR STATING WHEN TREATMENT OR REPAIR WILL NOT BE COVERED BY THE CONTRACT
• Repair contracts will not cover repairs from termite damage under every condition. The contract must state when retreatment or
repair will be done, and conditions under which the company can refuse to retreat or repair,
• These conditions have to be stated and be under headings in the contract that are in bold print. Companies typically refuse repair or
retreatment if the condition of the house is such that moisture or leaks result in termite infestation, or where siding makes it hard to see
termite infestation
Examples of this are:
• Cracks in concrete slabs
• Wood or wall siding in contact with ground
• Plumbing leaks
• Leaks in the roof
• Water accumulating against side of house
The law does require that companies notify you if they see conditions which would void the repair promise and they have to give you a
chance to correct the condition before voiding the contract or denying repair coverage.
• Contracts may have a condition that does not cover Formosan termite damage until a specific time period has passed. This means
that if damage occurs during this period the company will not pay for repair.

gives you the best contract options.
