Before you decide to upgrade your apartment(other than paint or carpet) you must obtain
permission from the Board of Directors and/or Miami Shores Village.
A copy of plans, specifications and permits, and a description of the licensed work to be
performed must be submitted for consideration and approval by the Miami Shores Village
Building Department(305-795-2204)
It is the owner's responsibility to ensure that the contractor removes all excess construction
material or building debris. It cannot be placed in the dumpsters.
** Window frames must be gray in color to look like aluminum. Windows must be Two(2)
panels over Two(2) panels. Glass must be clear color.
I, as the unit owner acknowledge responsibility for any damage to the building or personal
injuries that may occur during this project. The Shores Condominium Inc. its officers and
employees are in no way responsible for damage or theft to my apartment or my belongings. (A

Florida Law requires Workers' Compensation insurance coverage under Chapter 440 of the Florida Statutes. Fla. Stat. § 440.05
allows corporate officers in the construction industry to exempt themselves from this requirement for any construction project prior to
obtaining a building permit. Pursuant to the Florida Division of Workers' Compensation Employer Facts Brochure:
An employer in the construction industry who employs one or more part-time or full-time
employees, including the owner, must obtain workers' compensation coverage. Corporate officers
or members of a limited liability company (LLC) in the construction industry may elect to be
exempt if.
1. The officer owns at least 10 percent of the stock of the corporation, or in the case of
an LLC, a statement attesting to the minimum 10 percent ownership;
2. The officer is listed as an officer of the corporation in the records of the Florida
Department of State, Division of Corporations; and
3. The corporation is registered and listed as active with the Florida Department of
State, Division of Corporations.
No more than three corporate officers per corporation or limited liability company members are
allowed to be exempt. Construction exemptions are valid for a period of two years or until a
voluntary revocation is filed or the exemption is revoked by the Division.
Your contractor is requesting a permit under this workers' compensation exemption and has acknowledge that he or she will not use
day labor, part-time employees or subcontractors for your project. The contractor has provided an affidavit stating that he or she will
be the only person allowed to work on your project. In these circumstances, Miami Shores Village does not require verification of
workers' compensation insurance coverage from the contractor's company for day labor, part-time employees or subcontractors.

General Conditions
General conditions define each contracting party's duties and
privileges, and the rules by which they are to meet obligations and
perform the work they agreed upon. so this means daily cleanup of
entire area.

40% when kitchen is installed and permit is final
Except as otherwise set forth within the terms of this Agreement, any additional
equipment or services required by authorities having jurisdiction over the Work
(including, without limitation, governmental, association, or otherwise) will be
considered a change of scope and will be quoted as an additional charge including,
but not limited to, site plans, surveys, plan processing and/or permit costs which are
not included in the Total Price above. The subtotal, labor and service charge
' Wormational and estimate purposes only and Customer agrees to pay the Total Price stated above
regardless of the actual costs to Contractor of any such supplies, materials and/or labor. Except as
otherwise provided herein, the Contractor shall provide and pay for all licenses, labor, bonds, materials,
equipment, tools, construction, equipment, machinery, transportation, other facilities and services
necessary for the proper execution and completion of the Work.
ACCEPTANCE OF AGREEMENT
The above Total Price, specifications, and any incorporated conditions of this Agreement are hereby
accepted by Client and Contractor. Contractor (or Supervising Contractor) as the case may be, is hereby
