The undersigned has been commissioned by the owner to inspect for the recei tification a building
located at 9534 NE 2 AVE Miami Shores, FL 331 38-2705 , as indicated in the attached report.
On June 9, 2018, an inspection was conducted at the above referenced property. The building was found
to be electrically safe for its use and present occupancy as per Florida building Code, Chapter 8c-3(f).
"As a routine matter, in order to avoid possible misunderstanding, nothing in this report should be
construed directly or indirectly as a guarantee for any portion of this structure". To the best of my
knowledge and ability, this report represents an accurate appraisal of the present condition of the
