Thank you for this opportunity to serve you. You are the reason we exist. Serving you is
our professional passion and vocation. We hope your experience has exceeded your
expectations. Your 40-year recertification process is almost completed. Here's where
you take over:
Please find enclosed two (2) original copies of the 40 year recertification reports
in the format required by the ordinance.
■ You will notice that each copy has a cover letter addressed to the building official.
The address for the building official on the cover letter is the location that the
report must be delivered to.
■ Please keep one original copy in a safe place for your records.
• Please submit the 2"d original report to the building official at the address
listed on the cover letter. We recommend you mail it certified with return receipt
or hand deliver it and obtain a receipt proving they have received the report.
• You must pay the building official office, city or county the amount stated on the
notification letter you received from them. They will not accept the report if it is
not accompanied by a check for this fee.
■ Some building departments may require you fill out a permit application. Please
reference your notification letter from the building official to determine if this is
required in your case.

for compliance with Section 8C-6 and determined the following:
( X ) The parking lot(s) is not adjacent to or abutting a canal, lake or other body of
water.
( ) The parking lot(s) is adjacent to or abutting a canal, lake or other body of water
and parked vehicles are protected by a guardrail that complies with Section 8C-6
of the Miami -Dade County Code.
( ) The parking lot(s) is adjacent to or abutting a canal, lake or other body of water
and parked vehicles are not protected by a guardrail that complies with Section
8C-6 of the Miami -Dade County Code. I have advised the property owner that
he/she must obtain a permit for the installation of the guardrail and obtain all
required inspection approvals no later than April 10, 2014.
