Morrison Hershfield has completed a post construction visual observation of the above referenced
project. The scope of work under permit # CC-07-19-1714 involved the removal and installation of
antennas and hybrid cables to an existing building rooftop.
This evaluation makes use of digital photos of the as -built conditions taken after construction. Based on
observations, I hereby certify that the installation of the structural components of the mounts and
antennas at the site referenced above has been performed in general conformance with the approved
plans, specifications and the Florida Building Code in effect.
To avoid any misunderstanding, I hereby state that, to the best of my knowledge, belief and professional
judgment, this report represents an accurate appraisal of Sprint's antenna installation, based upon
careful evaluation of observed conditions to the extent reasonably possible.
