inspection will not be approved and a reinspection fee will be charged.

I, Mark A. Campbell, having performed and approved the required inspections at the job
site. This letter will hereby attest that I have approved the installation of a new tie beam
with (4) # 5 bars above the existing window at the new covered porch area. The new 2X
ledger for the truss roof covered porch shall be attached with Y2" ox. expansion bolts.
There was no concrete structure at that location for the attachment of the new ledger to
support the roof trusses. See the attached sketch and highlighted notes.
