Perry Electric, Inc. hereby informs you that he has or is furnishing labor and/or materials to complete the work
indicated in this work order. Florida law prescribes for the serving of this notice and restricts your right to make
payments under your contract in accordance with S. 713.06, Florida Statutes.

If the customer herein, or the "Prime Contractor" (if applicable), become default in the payment to the Contrac-
tor, Oliver Perry Electric, Inc., then the Contractor may take steps to collect the monies due him by whatever
means necessary, including but not limited to; acquiring credit information about the customer, or; charging the
customer credit card(s), (Customer authorizes this by signing this agreement), or; liening the customer's property
on which the work was performed. Title to all materials, installed or stored, shall not pass to the customer
become default in the payment(s), then the Contractor shall have the right to remove said materials and shall be
held harmless for any damage done resulting from the removal thereof.
7patching & 'Repairing
It is understood that sometimes it is necessary to make holes, break sheetrock and/or plaster to conceal wires and
cables, and that all patching, repairing and painting is the responsibility of the customer. The contractor will not
be held responsible for any repairs, structural or otherwise, related to this work order.

If a permit is required to complete the work as described herein, it shall be acquired by the contractor with the
full cooperation of the customer. The cost of the permit(s) is in addition to the costs as represented herein, and
the time expended by the contractor to acquire the permits) will be billed to the customer at the prevailing rate.
The customer further agrees to make the premises available to the inspecting authority, so that the necessary
inspections can be made. Oliver Perry Electric, Inc. shall not be responsible for any existing building or electri-
cal code violations.

Notice to Owner - Workers' Compensation Insurance Exemption
Florida Law requires Workers' Compensation insurance coverage under Chapter 440 of the Florida Statutes. Fla. Stat. § 440.05
allows corporate officers in the construction industry to exempt themselves from this requirement for any construction project prior to
obtaining a building permit. Pursuant to the Florida Division of Workers' Compensation Employer Facts Brochure:
An employer in the construction industry who employs one or more part-time or full-time
employees, including the owner, must obtain workers' compensation coverage. Corporate officers
or members of a limited liability company (LLC) in the construction industry may elect to be
exempt if.
1. The officer owns at least 10 percent of the stock of the corporation, or in the case of
an LLC, a statement attesting to the minimum 10 percent ownership;
2. The officer is listed as an officer of the corporation in the records of the Florida
Department of State, Division of Corporations; and
3. The corporation is registered and listed as active with the Florida Department of
State, Division of Corporations.
No more than three corporate officers per corporation or limited liability company members are
allowed to be exempt. Construction exemptions are valid for a period of two years or until a
voluntary revocation is filed or the exemption is revoked by the Division.
Your contractor is requesting a permit under this workers' compensation exemption and has acknowledge that he or she will not use
day labor, part-time employees or subcontractors for your project. The contractor has provided an affidavit stating that he or she will
be the only person allowed to work on your project. In these circumstances, Miami Shores Village does not require verification of
workers' compensation insurance coverage from the contractor's company for day labor, part-time employees or subcontractors.
