The investigation of repairs was limited to the exterior elements of the building structure at this time. The existing concrete structural slab elements exhibited various rectangular openings that were cut into the slab at various areas. The slab is 12" in thickness and the existing reinforcing bars were cut along with the slab. The rebar is #6 at 6" on center on one opening and #5 bars at 12" on center for the other openings. The purpose of this report is to establish a procedure for recasting the concrete and restore the slab to its original design. The concrete openings would require splicing the rebar, repairing the visqueen membrane, compacting the soil to 95% density, and placing 5000 psi concrete. The surface texture is to match the existing surface of the adjacent concrete. We recommend, therefore, that the above repair procedure be implemented to repair the existing concrete slab openings All repairs shall be made in accordance with approved methods and procedures and per the manufacturers' material recommendations. In addition, the work must conform to the requirements of the 2017, 6th Edition, Florida Building Code as well as the American Concrete Institute (ACI). Work under this permit application will not reduce the structural strength of the building or any member thereof.
