Important Notes and Disclosures:
1. Permit fees, licenses, insurance, trash hauling charges, taxes, and A-1 Warranty are INCLUDED in price.
2. Variations, deflections, humps, irregularities, dips, etc are fairly common on roof decks. Roof coverings will conform to existing deck
variations. If the existing deck is not flat and level this will be evident in the finished roof.
3. A certain amount of waviness referred to as "Oil Canning" is inherent in any light gauge sheet metal product. The amount of visible Oil Canning
can vary due to many factors — time of day, width of sheet metal, weather conditions, evenness of substrate, etc. Oil Canning does not affect
the structural integrity of standing seam panels and is not sufficient cause for rejection.
4. We are not responsible for damage done to our completed work by other trades.
5. Customer is responsible to notify in writing the location of exposed sheathing, septic tank, and drain field.
6. No mechanical, electrical, HVAC, roofing, woodwork, deck work, metal work, plastering, stucco, painting, caulking or any other unspecified
item is included in this contract unless written in the specifications elsewhere in this document. Roof Gas Vent inspection/permitting is not
included.
7. Florida State Hurricane Mitigation requirements may affect scope of work. All reroofs of properties that have a building value that exceeds
$300,000 (tax ad valorem) are required to retrofit the roof to wall connections to meet current code. Proposal does not include this work or
any other hurricane mitigation related work that is not in specifications.
IMPORTANT NOTE: Reroofing is the most cost effective time to upgrade or enhance your roof to wall strapping if it is substandard. Your
insurance premium may be affected by the roof to wall strapping conditions on your home. Even though the Mitigation code may not require a
strapping upgrade it might be in your interests to check with your insurance agent for advice with regard to this premium reduction item.
8. Unless otherwise stated payments are to be made as per terms dictated in this document; in the event payment is not made as stated herein,
all fees incurred in collection, such as attorney fees, court costs, filings and collection agencies are to be paid by the owner. Any unpaid
balance will accrue interest at a rate of 18% APR from due date amortized daily.
Job Assumptions:
Your A-1 representative/estimator has carefully considered all the conditions that affect the performance of your new roofing or waterproofing
system. Unless otherwise stated elsewhere in this proposal, the following assumptions are presumed to be valid for the performance of this
proposal. If for whatever reason these assumptions are found to be otherwise, please inform your representative as additional charges may apply.
1. A tasteful municipality approved job sign is permitted to be posted on site upon acceptance of proposal and until final payment.
2. Debris removal trailer, dump truck, container, etc., is able to be positioned alongside building within 5-10 feet of roof deck work area. All
driveway areas are available while work is underway.
3. City Electricity and Water are active and accessible from outside.
4. There are no open beam sections of the roof that are above interior spaces or garages.
5. Material deliveries can be made directly to roof deck from delivery vehicle conveyor.
It is our intention to protect your home or building to the very best of our ability. We understand the care and concern that every owner
experiences for their property. However much we perform our work in a manner designed to mitigate inconvenience and damage to occupants
and building, roofing work is unavoidably messy, noisy and physically stressful on a structure. Unless specifically stated elsewhere in this proposal,
please be aware of the following conditions as we cannot be responsible for cleaning or damages beyond our control.
1. We recommend that prior to start of the job all fragile or irreplaceable items on interior and exterior walls and ceilings that could be
disturbed and/or damaged during the job, be removed or securely fastened in place.
2. Open Beam areas and items stored in the attic: Please remove or cover vehicles, furniture, floors, items, etc.
3. Exterior Gutters, Downspouts, Antennas, Cables, Satellite Dishes, Special Electronics, etc.: Please have them professionally removed if it is
possible or likely that they will be inadvertently damaged or affected during the performance of the work.
4. Existing construction conditions: Hidden substandard or previously damaged or compromised construction elements such as: driveways,
roof decking, structural elements, ceilings, exterior tile surfaces, siding, etc.,

industry standard roofing practices:

1524.1 Scope. As it pertains to the section, it is the responsibility of roofing contractor to provide the owner with
the required roofing permit, and to explain to the owner the content of the section. The provisions of Section R4402
govern the minimum requirements and standards of the industry for roofing system installations. Additionally, the

INSTALLATION:
1. VersaShield® Fire -Resistant Roof Deck Protection or VersaShield® Solo— Fire -Resistant Slip Sheet shall be
installed in strict compliance with applicable Building Code.
2. VersaShield® Fire -Resistant Roof Deck Protection or VersaShield® Solo -Fire -Resistant Slip Sheet shall be
installed with a minimum 4 -inch head lap in a shingle layer fashion.
3. VersaShield® Fire -Resistant Roof Deck Protection shall be mechanically fastened with approved fasteners in
compliance with Florida Building Code 1518.2 when used in lieu of a code prescribed ASTM D 226 Type H
underlayment.

LE IITATIONS:
1. Fire classification is not part of this acceptance, refer to a current Approved Roofing Materials Directory for fire
ratings of this product
2. This acceptance is for prepared roofing applications. Minimum deck requirements shall be in compliance with
applicable building code.
3. VersaShield® Fire -Resistant Roof Deck Protection shall not be left exposed as a temporary roof for longer than 30
days of application. VersaShield® Solo— Fire -Resistant Slip Sheet shall not be left exposed to the weather.
4. VersaShiel& Fire -Resistant Roof Deck Protection and VersaShield® Solo- Fire -Resistant Slip Sheet are a
component used in roof assemblies. Roof assemblies are approved under specific assembly Notice of Acceptance.
5. VersaShiel& Fire -Resistant Roof Deck Protection or VersaShield® Solo— Fire -Resistant Slip Sheet may be used
with any approved roof covering Notice of Acceptance listing. VersaShield® Fire -Resistant Roof Deck Protection
or VersaShield® Solo- Fire -Resistant Slip Sheet may be used as a component part of an assembly in the Notice of
Acceptance. If VersaShield® Fire -Resistant Roof Deck Protection or VersaShield® Solo— Fire -Resistant Slip Sheet
is not listed, a request may be made to the Authority Having Jurisdiction (AHJ) or the Miami -Dade County Product
Control Department for approval provided that appropriate documentation is provided to detail compatibility of the
products, wind uplift resistance, and fire testing results.
6. VersaShield® Fire -Resistant Roof Deck Protection and VersaShield® Solo— Fire -Resistant Slip Sheet are a non -
asphaltic underlayment that may be used as a fire barrier for prepared roof assemblies. For fire classification of
specific roof assemblies using VersaShield® Fire -Resistant Roof Deck Protection or VersaShiele Solo— Fire -

materials. The documentation submitted has been reviewed and accepted by Miami -Dade County RER -
Product Control Section to be used in Miami Dade County and other areas where allowed by the
Authority Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami -Dade County Product
Control Section (In Miami Dade County) and/or the AHJ (in areas other than Miami Dade County)
reserve the right to have this product or material tested for quality assurance purposes. If this product or
material fails to perform in the accepted manner, the manufacturer will incur the expense of such testing
and the AHJ may immediately revoke, modify, or suspend the use of such product or material within

WOOD DECK SYSTEM LIMITATIONS:
I A slip sheet is required with GAFGLAS® Ply 4 and GAFGLAS® F1exPly" 6 when used as a
mechanically fastened base or anchor sheet.
2. Minimum '/." DensDeck® Roof Board or %z" Type X gypsum board is acceptable to be installed
directly over the wood deck.
GENERAL LIMITATIONS:
1. Fire classification is not part of this acceptance; refer to a current Approved Roofing Materials
Directory for fire ratings of this product.
2. Insulation may be installed in multiple layers. The first layer shall be attached in compliance with
Product Control Approval guidelines. All other layers shall be adhered in a full mopping of
approved asphalt applied within the EVT range and at a rate of 20-40 lbs./sq., or mechanically
attached using the fastening pattern of the top layer
3. All standard panel sizes are acceptable for mechanical attachment. When applied in approved
asphalt, panel size shall be 4'x 4' maximum.
4. An overlay and/or recovery board insulation panel is required on all applications over closed cell
foam insulations when the base sheet is fully mopped. If no recovery board is used the base sheet
shall be applied using spot mopping with approved asphalt, 12" diameter circles, 24" o.c.; or strip
mopped 8" ribbons in three rows, one at each side lap and one down the center of the sheet allowing
a continuous area of ventilation. Encircling of the strips is not acceptable. A 6" break shall be
placed every 12' in each ribbon to allow cross ventilation. Asphalt application of either system shall
be at a minimum rate of 12 lbs./sq. Note: Spot attached systems shall be limited to a maxivIutip
design pressure of -45 psf. •
5. Fastener spacing for insulation attachment is based on a Minimum Characteristihgfc AF') value Cf.
275 lbf., as tested in compliance with Testing Application Standard TAS 105. If.tbe.fastener 'value,
as field-tested, are below 275 lbf insulation attachment shall not be acceptable....:..

7. Perimeter and comer areas shall comply with the enhanced uplift pressure requirements of their:..'

compliance with Roofing Application Standard RAS 117. Calculations prepared, signed and sealed
by a Florida registered Professional Engineer, Registered Architect, or Registered Roof Consultant
(When this limitation is specifically referred within this NOA, General Limitation #9 will not
be applicable.)
8. All attachment and sizing of perimeter milers, metal profile, and/or flashing termination designs
shall conform to Roofing Application Standard RAS 111 and applicable wind load requirements.
9. The maximum designed pressure limitation listed shall be applicable to all roof pressure zones (i.e.
field, perimeters, and comers). Neither rational analysis, nor extrapolation shall be permitted for
enhanced fastening at enhanced pressure zones (i.e. perimeters, extended corners and comers).
(When this limitation is specifically referred within this NOA, General Limitation #7 will not
be applicable.)
10. All products listed herein shall have a quality assurance audit in accordance with the Florida
Building Code and Rule 61G20-3 of the Florida Administrative Code.
