Pursuant to the authorization of Fernando J Yanez, the owner, by myself,
conducted an inspection at the above referenced site and scanned the structure and its
components with a "PROFOMETER 5" (BY PROSEC) in order to determine its
compliance with the requirements of the "Florida Building Code 2017". The date when
the construction was done in October 30 2018, My statement is based on the following
detailed, methodology procedure:
Inspections and Methodology: Existing Framing -Insulation -Drywall and existing
Filled Cell (Tie Downs) for Doors, Windows and openings that were closed with cmu

I-) Inspection for Existing Framine-Insulation-Drywall.":
A-) Partition Wall Drywall: Opened drywall to verify size and spacing of studs of the
partitions. Existing metal Studs 3-5/8" @ 16" o.c. with %i" Drywall in both sides w/16d
nails @ 16" anchor to studs. Existing 3" metal to the top track anchor to celling w/16d
nails @ 4" and bottom track anchor 3/16" tapcons to concrete slab @ 16" o.c. Inside to
the partition existing Insulation fi-foil vapor shiedl AA2 R=5.1, 125 Sq Ft Single Faced
Reflective Roll Insulation (24 -in W X 62.5 Ft L).

B-) Filled Cell (Tie Downs) : Existing (1) #5 Filled Cell (Tie Downs) in each corner for
the doors, windows and in the openings that were closed with cmu 8", Are connected to
the bottom to the existing footing with a depth of 4 "and connected to top to the existing
concrete tie beam with a depth of 6 ". Via Profometer and visual inspection.
According to Code FBC 2017.
For this reason, I Emiliano Orozco confirms that the property inspected at 142 Ne 103
Street, Miami Shores, Fl. 33138-2329., comply with the Code FBC 2017, ( Oh Edition).

Dear Building Official:
The Scope of Work performed in this house consisted of°#he replacement of the
kitchen cabinets and the remodeling of one full bathroom and one half bathroom.
The work was performed and the required inspections were all called for except
Building. Electrical and Plumbing final passed but the Building Inspector required
an engineering affidavit to verify that the drywall was correctly installed over the
existing wood studs in the 2 remodeled bathrooms.
With that purpose in mind, I visited the 2 bathrooms and located magnets over
the drywall screws to verify their location, and measured the distances between
magnets. This was done in lieu of destructive testing which is messy and
expensive just to reach the same conclusion. Please see the pictures taken.
To the best of my knowledge, belief and professional experience, the drywall
screws were installed following the requirements of the Florida Building Code
2017, Sixth Edition and that the studs have the correct and proper separation.

Dear Building official:
I, Yan Luis Solis P.E, did a site inspection for the above reference location. I attest that the
compaction at the Paver and Concrete area located at the East Side of the Property (Non vehicular
Traffic Courtyard) was more than 98% as per Note on the approved PWK-1 Plan. This compaction
was made in 2 different times in Stratum of 8" each with enough water helped giving us at least a
capacity of 2000 Pounds / Sq. Ft.
Concrete area compaction was field verified on April 29, 2019 and compaction on the paver area
on May 20, 2019.The material used was lime rock that has the optimum moisture content and
proper loose consistency after being compacted to a density no less than 98% determined by A
ASHTO T 180.
In each stratum preparation was the same and the material was placed in layer of no more than 8".
We checked different points approximately 4'-0" straightedge laid parallel and across and we did
not detected irregula► ities greater than 0.25".
The area was checked after compacted.
