SUBJECT: TASK ORDER NO. 1— Hurricane Irma DR 4337 — City 2017 -003, "Disaster Debris
Removal, Hazard Limbs and Trees Removal, Debris Management site operations.

SCOPE OF WORK: Task Order No. 1 authorizes Looks Great Services of MS, Inc. (LGS) to perform
Emergency Debris Removal and Removal of Debris hazards (hazard limbs, and tree) per Miami Shores
Village Mutual Aid agreement with City of Florida City, Florida (Florida City) and per proposal reply to
Florida City RFP 2017 -003 awarded to LGS September 12,2017. "LGS" will initiate and mobilize for
Emergency Debris Removal Services support as required to remove the immediate threat to health and
welfare of the Miami Shores Village community.
This Task Order No. 1 provides a Notice to Proceed with the following Scope of Work per LGS proposed
Scope of Services and Rates per proposal reply to "The City" Proposal dated September 7, 2017.
PAYMENT TERMS: Task Order #1 is for the first 60 Days of Emergency Services per Florida
Governor Rick Scotts Executive Order and per Federal/ FEMAs Declaration DR4337. LGS is to submit
invoices upon completion with the Emergency Debris Removal tasks the Miami Shores Village with
supporting FEMA required information per invoice.
SCHEDULE: Time is of the Essence and DP &O will initiate work immediately under this Task Order.
This task order is limited to 60 days from the mobilization date of September 21,2017 to support
Emergency Debris Removal Services as a result of Hurricane Irma DR 4337 per request of Miami Shores
Village.
