We, Jacki Alexander and Ido Alexander, owners of property located at 29 NE 91 Street in Miami
Shores, FL, have decided to not proceed with permit #s DEMO-4-16-1124 and RC-6-15-1597. No
work has been undertaken in connection with permit numbers. We the homeowners are now seeking
to sell the property and are under contract to do the same. Please see attached permit cancellation
form. At your earliest convenience, please let us know when verification of these permit cancellations
have taken place. Also, please note that all fees associated with the permit cancellations have been
paid by Ido Alexander on September 20, 2018.
