SFM understands the “Scope of Services” The Village has provided in the RFP documents and is committed to perform the debris reduction and disposal services in a positive and timely manner. The concept behind SFM Services, Inc. is a "one stop source" for public works or property managers to outsource by contracting "Professional Facility Support Services" which include:  Emergency Debris Removal/Disposal Services  Landscape Services  Janitorial Services SFM also provides disaster recovery services for other major public facilities, government entities and commercial properties throughout South Florida year round. SFM currently has a large presence in the disaster debris removal and disposal services throughout South Florida with a proven verifiable track record. SFM does not have any outstanding litigation that would threaten the viability of the firm or the performance of the contract. Enclosed please find proof of insurance. SFM’s ownership Mr. Jose Infante and Christian Infante are both ISA (International Society of Arboriculture) Certified Arborists. Jose Infante is also a licensed commercial pesticide applicator and a FNGLA Certified Landscape Maintenance Technician. (See attached certificates) Additionally, SFM has 2 other ISA certified arborists on staff. The SFM team uses the horticultural best management practices possible. Respectfully Submitted, Christian Infante Christian Infante President 3 Key Personnel SFM’s team is composed of highly motivated, trained, and experienced personnel. The SFM team has the following certifications and credentials: Jose M. Infante, President of SFM Services, Inc. has forty (40) years of experience in the landscape industry. He is also a certified arborist. Mr. Infante is qualified and experienced in all aspects of landscape services. He is also a FNGLA Landscape Maintenance Technician. Mr. Infante was a past chair of the APWA (American Public Works Association). He also holds a pest control applicator license. Christian Infante, Vice President has fifteen (15) years of experience in landscape management & irrigation. Mr. Infante has a Bachelor’s degree in Business Marketing & Management from Florida International University (FIU), an ISA Certified Arborist, and a certification in Horticulture and M.O.T. traffic control Mario Cantero, Landscape Manager. Mr. Cantero has been oversees all landscape operations. He has nine (9) years of experience in the service industry. He is M.O.T. certified and certified in Horticultural Studies. Mr. Cantero is currently seeking certification as Arborist with the International Society of Arboriculture. Mario Lignarolo, Vice President of Business Development has twenty (20) years of experience in related management. During 2005 and 2006, Mr. Lignarolo ran all the day- to-day operations of SFM Services, Inc. in Mississippi post Hurricane Katrina. During Disaster Recovery projects Mario assigns crews and proper equipment (bucket trucks, stump grinders, self-loaders, etc.) accordingly to each municipality. Robert Fisk, Environmental Planner has a bachelor degree in landscape architecture from Louisiana State University. Mr. Fisk is also an ISA certified arborist and has over 15 years of landscape & irrigation experience. SFM Maintenance Crews, Management strongly believes in promoting from within our own staff. All supervisors have begun at the maintenance personnel level. SFM has very low turnover compared to other firms in the industry.  Certified Arborists  M.O.T. Therefore for any Emergency, SFM is prepared to deploy and facilitate quickly and efficiently. Some of our clients in disaster recovery services include: • Miami-Dade County • Dade County School Board • Florida Dept. of Transportation • City of Doral • Town of Miami Lakes SFM’s additional crews can be dispatched to: • Install hurricane shutters • Debris removal & disposal • Supply generators, 2-way radios, etc. • Emergency water extraction & restoration • Trim low hanging dangerous branches SFM has provided disaster recovery service to several municipalities after the following storms: • Hurricane Andrew • Hurricane Wilma • Hurricane Katrina (Mississippi & Florida) • Hurricane Francis • Hurricane Charley • Hurricane Dennis • Hurricane Ike (Texas) • Hurricane Irma 5 Hurricane Recovery Equipment Supply and transport up to 50 gallons of fuel for emergencies. Bucket trucks available to trim any dangerous branches. Excavator with chipper attachment to mulch collected debris. Front end loaders to perform the “1st pass” through main artery roads. Self-loader grapple truck to pick up debris from right-of-ways. Tub Grinder for debris reduction operations. 6 2. Operations Plan Site Preparation Upon notice to proceed, SFM will make the necessary preparations to the TDSRS to facilitate our equipment and crews for its operations. The debris reduction area will have the appropriate setbacks and safety zones; appropriate signs, cones, safety barriers and caution tape identifying safety zones; a first aid station; site security and fire prevention measures; roofed inspection tower and temporary restrooms; scissor lift and generator. Mobilization Plan Upon notice to proceed, SFM Services, Inc. will SFM will mobilize equipment and crews to the (TDSRS) in the Village of Miami Shores. Mobilization will be 100% completed within 48 hours of the notice to proceed. The mobilization of equipment and personnel will be based on the quantity and types of debris assessed by SFM Services and agreed upon by the Village representative. Debris Reduction Collected debris will be reduced by grinding. This involves using an industrial tub or horizontal feed grinders powered by up to 1000 hp diesel engines. A grinder will process up to 400 cubic yards of clean woody debris per hour, reducing the volume by approximately 4 to 1. Grinding operations are the preferred method of reduction of vegetative debris to accomplish environmental resource conservation through recycle/salvage of wood chips. Although this operation is preferred for environmental purposes, it is, however, the most time consuming and costly reduction operation. Grinding/Chipping of construction and demolition (C&D) materials is prohibited. All personnel involved in this operation will be briefed on safety and will be equipped with fire extinguishers. A tractor with blade or a rubber tire loader will pick-up, and stock pile chips for temporary storage. Chips will be loaded out and hauled to a final disposal site as quickly as possible to reduce the threat of a conflagration. Water Trucks SFM Services, Inc. will have the required number of water trucks stationed at each debris- staging site. Water trucks will be utilized to reduce the threat of fire from all types of debris. Water trucks will also be used to dampen temporary roadways, to suppress dust from trucks entering and leaving the debris-staging area. 7 Site Maintenance SFM will maintain the TDSRS as necessary to ensure safety and minimize environmental impact. Normal maintenance will include: adding rock to roads for stabilization, modifying road system as needed, installing silt fences or berm systems to divert rainwater and protect areas from runoff and loss of topsoil, replace liners under stationary equipment, document and rectify any accidents, spills, or environmental mishaps that occur, Additionally, we will supply site security and appropriate fire prevention measures when necessary. Debris Disposal All debris collected by SFM during disaster recovery activities for Village will be disposed of in accordance with all applicable state, federal and local laws, codes, standards, and regulations. Mulch from woody debris reduced by grinding will be disposed of in a licensed landfill. Site Closeout & Restoration The TDSRS will eventually be cleared of all debris and restored to its pre-disaster condition and use. Our basic closeout steps are: remove all debris, stored material, and unnecessary equipment from the site. Closeout and restoration is generally completed within 30 days of receiving the last load of incoming debris. 8 3. Cost Proposal Debris Reduction Debris Reduction of vegetative debris via grinding at the Miami Shores Village permitted temporary debris storage reduction site (TDSR). Price includes set-up, equipment mobilization and demobilization, and final load out of debris; maintenance of onsite entry and exit roads, and closure of temporary debris storage and reduction site (TDSR). Price includes maintaining dust particulates to a minimum and provide dust particulate control if needed. Per Cubic Yard $ 4.50 Final Haul and Debris Disposal at a Class I Landfill Note: contractor is to provide a permitted disposal facility location which contractor has disposal contract price for reduced vegetative storm debris disposal.
