Dear Mr. Naranjo:
Pursuant to a request by management, the undersigned visited the above referenced site
on July 25, 2014 to review a structural condition located at the prefabricated wood trusses
support at the south end of the building. The support was altered in order to allow the
prefabricated wood trusses to fit the condition that existed in the field.
We requested to have additional wood members be added at this location in order to
compensate for this alteration. As a result and after the remedial action noted was
completed we find the existing condition to be structural acceptable.
We hereby state that to the best of our knowledge, belief and professional judgment those
portions of the structure outlined above meet the intent of the Florida Building Code and
are in substantial accordance with the approved plans.

Description
InsulFoam SP is an advanced roof insulation consisting of a
superior closed-cell, lightweight and resilient expanded poly-
styrene (EPS) with a durable and stable, factory-laminated
fiber glass facer. InsulFoam SP has a nominal density of
1.25 Ib/ft' (pcf), and meets or exceeds the requirements of
ASTM C578,Type VIII, Standard Specification for Rigid, Cel-
lular Polystyrene Thermal Insulation. In addition, InsulFoam
SP offers a long-term, stable R-Value and has excellent di-
mensional stability, compressive strength and water resistant
properties.
Uses
InsulFoam SP is specifically designed for low-sloped roof
applications that employ mechanically fastened or ballasted
TPO, PVC, EPDM and CSPE, as well as low-sloped built-
up, modified bitumen and fully adhered single ply roofs that
incorporate cover boards or slip sheets. Please consult lo-

Duro-Last roofing membrane and roofing systems have been classified and listed by Underwriters Laboratories (UL)
and Underwriters Laboratories of Canada (ULC). These listings include systems rated for protection from external fire
exposure, uplift resistance and impact resistance. Duro-Last membrane is also listed in ceiling-roof designs evaluated
for performance during internal fire exposures shown below as"Roofing Membranes (CHCI /United States)". Duro-
Last, Inc. is authorized to use the appropriate UL Mark and is subject to UL's Follow-Up Service to ensure that the
roofing materials and production procedures continue to comply with UL's requirements.
The roofing material and system listings can be found in the"Roofing Materials& Systems Directory" published by UL
and can also be found on UL's website (www.ul.com)within the Online Certifications Directory. For Canada, the

level of protection is indicated by the classification (A, B or C) shown for each system. Roofing systems investigated
for impact resistance are so indicated in the individual classifications by the appearance of the word"Impact" and the
applicable Class Number(3 or 4). The basic standards used to investigate products in this category are ANSI/UL790
"Test for Fire Resistance of Roof Covering Materials" and UL2218"The Standard for Impact Resistance of Prepared
Roof Covering Materials." For Canada, the standard is CAN/ULC-S107"Method of Fire Tests for Roof Coverings."

CONSIDERATIONS
R4402.13.1 Scope. As it pertains to this section,it is the responsibility of the roofing contractor to provide the
owner with the required roofing permit,and to explain to the owner the content of this section.The provisions of
Section R4402 govern the minimum requirements and standards of the industry for roofing system installations.
Additionally,the following items should be addressed as part of the agreement between the owner and the
contr .The owner's initial in the adjacent box indicates that the item has been explained.
1.Aesthetics-Workmanship:The workmanship provisions of Section R4402 are for the purpose of
ding that the roofing system meets the wind resistance and water intrusion performance standards.Aesthetics
(appearance)are not a consideration with respect to workmanship provisions.Aesthetic issues such as color
or architectural appearance,that are not part of a zoning code,should be addressed as part of the agreement
be he owner and the contractor.
2.Renailing Wood Decks:When replacing roofing,the existing wood roof deck may have to be
ailed in accordance with the current provisions of Section R4403.(The roof deck is usually concealed prior

3. Common Roofs: Common roofs are those which have no visible delineation between

2. insulation may be installed in multiple layers. The first layer shall be attached in compliance with
Product Control Approval guidelines. All other layers shall be adhered in a full mopping of
approved asphalt applied within the EVT range and at a rate of 20-40 lbs./sq., or mechanically
attached using the fastening pattern of the top layer
3. All standard panel sizes are acceptable for mechanical attachment. When applied in approved
asphalt, panel size shall be 4'x T maximum.
4. An overlay and/or recovery board insulation panel is required on all applications over closed cell
foam insulations when the base sheet is fully mopped. if no recovery board is used the base sheet
shall be applied using spot mopping with approved asphalt, 12" diameter circles, 24" o.c.; or strip
mopped 8" ribbons in three rows, one at each sidelap and one down the center of the sheet allowing
a continuous area of ventilation. Encircling of the strips is not acceptable. A 6" break shall be
placed every 12' in each ribbon to allow cross ventilation. Asphalt application of either system shall
be at a minimum rate of 12 lbs./sq.
Note: Spot attached systems shall be limited to a maximum design pressure of-45 psf.
5. Fastener spacing for insulation attachment is based on a Minimum Characteristic Force (F')value of
275 Ibf, as tested in compliance with Testing Application Standard TAS 105. If the fastener value,
as field-tested, are below 275 Ibf insulation attachment shall not be acceptable.
6. Fastener spacing for mechanical attachment of anchor/base sheet or membrane attachment is based
on a minimum fastener resistance value in conjunction with the maximum design value listed within
a specific system. Should the fastener resistance be less than that required, as determined by the
Building Official, a revised fastener spacing, prepared, signed and sealed by a Florida Registered
Engineer,Architect, or Registered Roof Consultant may be submitted. Said revised fastener spacing

1. Please insure the roof deck is clean, smooth, and dry prior to beginning installation.
2. REX 'SynFelt is laid out horizontally(parallel)to the eave with the printed side up,with 3"(76
mm)horizontal laps and 6" (152 mm)vertical laps. On roof slopes less than 3:12, REX'" Syn-
Felt should be lapped at 24" (61 cm) or to center line as printed, over the underlying course.
3. REX-SynFelt should be attached to the roof using plastic cap roofing nails having a 1" (25mm)
diameter plastic cap. Spacing shall be ate,, (20 cm)on center at both horizontal and verti-
cal caps for normal wind zones. In high wind zones and coastal areas, REX 'SynFelt shall be
fastened at 4" (101 mm)at both horizontal and vericle laps. In all cases fasten at 24"(61 cm)
on center along the center of the roll in the field of the roof. The pre-printed nail pattern is to
the spacing as described above for both normal as well as high wind conditions.
4. The use of staples to attach REX-SynFelt is permitted only when the final roof covering is to
be installed immediately afterward. Pneumatic staple guns are not recommended.

LIMITATIONS:
1. Fire classification is not part of this acceptance, refer to a current Approved Roofing Materials Directory for fire
ratings of this product
2. This acceptance is for prepared roofing applications. Minimum deck requirements shall be in compliance with
applicable building code.
3. VersaShield®Fire-Resistant Roof Deck Protection shall not be left exposed as a temporary roof for longer than 30
days of application. VersaShield®Solo?"' Fire-Resistant Slip Sheet shall not be left exposed to the weather.
4. VersaShield® Fire-Resistant Roof Deck Protection and VersaShield' SoloT"' Fire-Resistant Slip Sheet are a
component used in roof assemblies. Roof assemblies are approved under specific assembly Notice of Acceptance.
