Notice to Owner - Workers' Compensation Insurance Exemption
Florida Law requires Workers' Compensation insurance coverage under Chapter 440 of the Florida Statutes. Fla. Stat. § 440.05
allows corporate officers in the construction industry to exempt themselves from this requirement for any construction project prior to
obtaining a building permit. Pursuant to the Florida Division of Workers' Compensation Employer Facts Brochure:
An employer in the construction industry who employs one or more part-time or full-time
employees, including the owner,must obtain workers' compensation coverage. Corporate officers
or members of a limited liability company (LLC) in the construction industry may elect to be
exempt if:
1. The officer owns at least 10 percent of the stock of the corporation, or in the case of
an LLC,a statement attesting to the minimum 10 percent ownership;
2. The officer is listed as an officer of the corporation in the records of the Florida
Department of State,Division of Corporations;and
3. The corporation is registered and listed as active with the Florida Department of
State,Division of Corporations.
No more than three corporate officers per corporation or limited liability company members are
allowed to be exempt. Construction exemptions are valid for a period of two years or until a
voluntary revocation is filed or the exemption is revoked by the Division.
Your contractor is requesting a permit under this workers' compensation exemption and has acknowledge that he or she will not use
day labor,part-time employees or subcontractors for your project.The contractor has provided an affidavit stating that he or she will
be the only person allowed to work on your project.In these circumstances,Miami Shores Village does not require verification of
workers' compensation insurance coverage from the contractor's company for day labor,part-time employees or subcontractors.

estate and personal property taxes for the year 2017,which are not yet due and payable.

The undersigned for and in consideration of the closing/title agent, Kellermann Varela PL (the "Closing Agent")
this day disbursing the funds for the closing of the transaction(the "Closing")agree, if required by Closing Agent
to fully cooperate and adjust for clerical errors on any or all closing documentation. Said adjustments to be made
if deemed necessary or desirable in the reasonable discretion of closing/title agent.
The undersigned further agrees to cooperate fully with Closing Agent in all efforts to assure that required sums
for closing are collected from the appropriate parties. Further, the undersigned agree that should any oversight or
error occur in the collection of said required sums they will immediately upon written or verbal notification make
any required corrections or pay additional monies required.
If requested by Lender(if any), Closing Agent, Title Agent or Title Underwriter, the undersigned agrees to fully
cooperate and adjust for clerical errors, including the execution or re-execution of any reasonable documentation
and/or the remittance of any additional sums.
In the event that Closing Agent in its efforts to correct documents or collect additional sums required to complete
this purchase transaction should incur legal, attorneys and/or court costs, the responsible party shall be
responsible to reimburse Closing Agent for said costs.
Sometimes recording fees and courier/express mail fees may vary due to the unknown amounts at the time of
closing. Therefore,the undersigned acknowledges hereto,that.monies collected for recording and courier/express
mail fees may be more or less than the amount-collected on the closing statement. Any shortfalls or overages
shall be considered the cost of doing business.Closing Agent will neither refund or collect said differences.
If the most recent property tax bill issued does not cover through the closing date,then the tax prorations set forth
on the settlement statement are based upon an estimate. The basis of proration as set forth on the settlement
statement is hereby accepted by the parties to this transaction. It is hereby understood and agreed that the actual
taxes, if different, will be adjusted between the parties upon demand. The parties will also resolve any further
taxes due by virtue of a later change or final certification of previous years' tax rolls, including a change from
unimproved to improved property. Closing Agent is not liable or responsible for adjustment or re-proration of
taxes. Closing Agent is not responsible or liable for additional taxes, other charges or tax refunds, if any, and
shall not be liable should any of the parties to this transaction fail or refuse to re-prorate the taxes.
The undersigned acknowledges that during the course of the settlement of funds interest may be earned in favor
of Kellermann Varela PL, and the undersigned hereby waives any rights to such interest.
The undersigned herein acknowledges responsibility for any sums accruing under said items prior to the date of
closing. Any sums due not shown on the closing statement will be resolved between the parties: (i) Any unpaid
trash pickup or trash removal fees, (ii) Any unpaid special assessment liens due the city or the county which are
not recorded under Official Records Book and Page and Clerk's File Number in the county records, (iii) any
unpaid utility bills, (iv) Any unpaid personal property taxes, and (v) Any special assessments or maintenance
liens due an association not recorded in the county records.
Closing Agent does not adjust or assume liability for charges for water, rents, gas, electricity, taxes on personal
property, garbage taxes or fees, license fees or taxes, service/maintenance contracts (pest control, appliance
maintenance, pool care, lawn care, alarm systems, etc.), association assessments or dues, or estoppel information
furnished by mortgagees or others. The settlement statement has been reviewed and approved and Closing Agent
is irrevocably authorized and directed to complete the closing of the transaction and make disbursement in
accordance therewith. In the event of mortgage assumption, if the undersigned has received a credit for the
escrow account balance, then the undersigned hereby assigns all right, title and interest in said account to Buyer.
