A RESOLUTION PROVIDING FOR THE REAFFIRMATION OF AFLAC
FLEXIBLE BENEFIT PLAN AGREEMENT FOR FY 2000-2001
WHEREAS,Miami Shores Village wishes to maintain the existing cafeteria plan within the
context of Section 125 of the Internal Revenue Code for the benefit of eligible Village employees;and,
WHEREAS,the Village has contracted for the aforementioned Flexible Benefits Section 125
Plan with The American Family Life Assurance Company of Columbus (AFLAC), Inc., last
negotiated in September, 1992; and,
WHEREAS,the carrier(AFLAC)requires Village Compliance with various Internal Revenue
Code provisions to continue to sponsor this voluntary benefits program for eligible Village employees.
NOW, THEREFORE, BE IT DULY RESOLVED BY THE MIAMI SHORES
VILLAGE COUNCIL,that:
1. The Village Manager and/or his designee are hereby authorized to reaffirm the
existing voluntary Flexible Benefits Plan with the American Family Life Assurance
Company of Columbus (AFLAC) comprised of the Adoption Agreement and Plan
Documents.
2. The Village Manager and/or his designee are hereby authorized, without a further
resolution, to execute the Adoption Agreement and all related documents or
amendments that may be required to retain compliance with federal, state and local
laws associated with the Section 125 Flexible Benefits Plan.
